Cleanup headers

* atomic_ -> uatomic (to remove namespace clash with libkcompat)
* moved arch.h, compiler.h, arch_uatomic.h to
  /usr/include/urcu/

to make sure we do not pollute system headers.

Also add call_rcu() documentation to README.
