Cleanup order semantic

[ Edit by Mathieu Desnoyers: renamed "end_order" to "last_order", to
  match the "first_order" inclusive semantic. Normally, "end" excludes
  the range limit value, but "last" includes it. We use an inclusive
  range end value here. ]
